What is an advance directive at Community Memorial Hospital?

An Advance Directive allows you to put your healthcare wishes and values in writing, as well as designate someone to make medical decisions for you if you are unable to do so yourself. You may also ask your doctor to complete a POLST form, which clarifies your wishes for end-of-life care.

Does URMC require two factor authentication?
Access to URMC VPN, systems and applications requires a URMC account and Duo two-factor authentication. To enroll in Duo, you must be connected to the URMC network and follow the instructions for the method you will use to verify yourself.
