How long is Einstein's mentorship program?
To give principal investigators the tools they need to become better mentors, Einstein is launching an eight-hour program for all faculty who have trainees.
Step 1
Provider signs and returns contract to Grisel Vazquez, Administrator to the Chair of the Department of Medicine. Credentialing cannot commence before receipt of a signed contract unless authorized by the Chair.
Step 2
Credentialing team meets with provider and reviews process, time frames, completes Medicare/Medicaid applications, etc.
Step 4
The Credentialing Manager notifies the Department, Division, and provider when the provider has received privileges.
Required Documentation
All documentation and paperwork must be submitted to Deborah Deluca-Parchen, Credentialing Manager.
Reappointments
CMO mails application to provider six months in advance of reappointment date.
