How do I access myfhn through the app?
Anytime you want to access MyFHN through the app, just log in with the same username and password set up for the online version of MyFHN. A Proxy Authorization Form is required to access the health information of anyone over age 12 by someone other than themselves.

FHN's new telemedicine app FHNow gives you access to a provider via smartphone, tablet, or desktop computer, and your virtual visit is just $59. Mobile Rx Pantry Program. FHN and the Northern Illinois Food Pantry are helping get a healthy "dose" of good food to those who need it.

A Proxy Authorization Form is required to access the health information of anyone over age 12 by someone other than themselves. Please note that all signatures must be witnessed by an FHN staff member.
How to sign up for FHN?
You can sign up for FHNow in a number of ways – choose what's easiest for you: Download the FHNow app (search for 'FHN Now') from the Apple App Store or Google Play and follow the signup instructions. Visit www.MDLive.com/FHNow and click "Get Started". Call toll-free 1-800-400-MDLIVE (6354) and choose option No. 1.
How much is virtual visit?
Virtual visits are $59, payable by credit or debit card. No insurance can be processed through the FHNow service; check with your insurance provider to see if the fee is reimbursable. The fee may qualify for reimbursement from HSA or FSA funds; check with your HSA/FSA provider for details.
Can you schedule an appointment with FHNOW?
Appointments are available immediately or can be scheduled in advance, and FHNow physicians can send prescriptions to area pharmacies. Lab work, x-rays and other services requiring in-person care are not available through FHNow, and children under 36 months with a fever must be referred for in-person care.
