To receive your access code by email or text message, simply call your Lifespan health care provider's office. You will receive a code immediately after answering five easy questions to confirm your identity. To receive your access code online, your identity will be verified through Experian, a leading global information services company.

How do I report an incident or request additional help with lifespan?

What is Life Chart EHR?

LifeChart is the new Lifespan electronic health record (EHR), replacing LifeLinks. It is a comprehensive platform, using Epic software, that allows us to share information seamlessly throughout our health care system, from our hospitals and clinics, to our ambulatory centers and community partners.
