You simply need to ensure that Mofitt is in-network for all employee plans; that other health benefits providers refer to Moffitt CONNECT; and that you agree to participate in communicating the program details to your employees.

Read the Telemedicine consent form prior to your appointment. You will be asked for your verbal consent at the beginning of your visit.
Step 2: Prepare for Your Virtual Visit Location
Download the Zoom app to access a secure connection from your PC computer or mobile device.
Step 3: Access Your Virtual Visit
Access your Moffitt Virtual Visit invite. Connect by selecting the hyperlink 5-10 minutes prior to your visit.
