How do I transfer my prescriptions from mosaic to another provider?
Mosaic Life Care must have record of care (i.e. medical record) AND The prescriber must be in Mosaic Life Care St. Joseph - Medical Center or a facility (i.e. child site) that is 340B eligible. If you wish to transfer any of your prescriptions to a 340B participating pharmacy, please contact your Mosaic Life Care provider.
