If you have received care at OhioHealth hospitals, outpatient centers, physician offices or urgent care centers, you may have an OhioHealth MyChart account. Ask any member of your care team for an activation code, or request one online.

What happened to myohiohealth patient portal?
OhioHealth is no longer supporting the myOhioHealth patient portal. We have transitioned to OhioHealth MyChart for the greater Columbus area, Delaware, Marion, Mansfield and Shelby, Ohio. OhioHealth MyChart offers the same access to preregistration and your medical records that you had with myOhioHealth.
When you need care today, choose an urgent care video visit
If you have COVID-19 concerns, or minor illnesses and injuries, skip the waiting room and stay comfortable at home. Same-day urgent care video visits can be scheduled online or by phone, with extended hours available. Log in to your MyChart account to access additional virtual urgent care appointment times, 9 AM-9 PM daily.
Talk with your doctor or get routine care through telehealth appointments
If you have an OhioHealth Physician Group primary care or specialist provider, you can visit with them by phone or video through telehealth appointments. It's safe, secure and, best of all, convenient. Self-schedule through OhioHealth MyChart, and show up in your sweats.
If your symptoms can wait, get secure medical advice through E-Visits
When you have questions about symptoms for your doctor that don't need answered right away, send them an E-Visit questionnaire through OhioHealth MyChart. They'll get back to you by the next business day, and you can continue going about yours.

Restrictions to in-person records requests
To protect the health of OhioHealth associates and visitors, we are changing our operations to restrict in-person visits to the Health Information Management department.
Fees
If requested for follow-up care, there are no fees for a copy of your medical record. There may be a fee when requested for other purposes (such as legal, insurance claims or personal use).
