What is an account request form?

Account Request Form - provides the Health Information Management team the information needed to provide an individual access to the Provider Portal.

Access and Confidentiality Agreement - the Access and Confidentiality Agreement defines the responsibilities and obligations of each party in regard to Penn State Health’s clinical information system. To be signed by one of the practice partners with signatory authority.

What is a provider list?

Provider List - outlines all providers (MD/DO/CRNP/PA/APCs) who are required to have their patients available on the Provider Portal. If your providers see patients at multiple locations, please add and identify those locations. If your practice utilizes direct secure email addresses, please also provide the provider’s direct secure email address. Direct secure email addresses allow us to also securely send you Patient Health Information (PHI). If you currently do not have direct secure email addresses and are interested, see your certified EHR software vendor. How to join the Health Portal?

If you registered during your office visit, you should receive an email "invitation" to join the health portal. The link in this email message will take you to your registration information. If you do not see the message in your email inbox, you may want to check the junk or spam folder to see if it is there.

How to create a Hershey account?

You can create your account online using the Self-Enroll process. Or you may sign up with the office staff during any visit to the Milton S. Hershey Medical Center or one of our Medical Group locations.

Can parents get a copy of medical records from Penn State?

As always, parents and guardians can contact their family physician to discuss their child's medical situation, and can contact Penn State Hershey Medical Records office to obtain paper copies of medical records by providing proper identification.

When can a child access their medical records?

On the child's 18th birthday , parental or guardian access to online medical records will be terminated. The child will be able to initiate their own account for access to their electronic health record during their next office visit. Federal HIPAA law requires privacy of health records at age 18.
