Penn State Health Provider Portal is a secure portal application that provides real-time, read-only access to your patient’s information from encounters with Penn State Health. All of the below documents must be completed and submitted to Penn State Health’s Health Information Management team at HIMProviderPortalGroup@pennstatehealth.psu.edu.
