What is accesshealth screening?

AccessHealth team members screen uninsured patients for enrollment based on household income and residency. If you or someone you know could benefit from assistance, contact us for an appointment to see if you qualify.

What is NHP in AccessHealth?

Once enrolled in AccessHealth, you may also enjoy the support of the Neighborhood Health Partners (NHP) Care Team. A team of community health workers, paramedics and social workers brings the help you need to where you live.

Why is AccessHealth important?

AccessHealth brings together a network of healthcare providers and community agencies focused on supporting the health and well-being of people who cannot afford to get care on their own.

Do you have to be part of AccessHealth to receive NHP?

Patients must be part of AccessHealth to receive NHP Care Team services. AccessHealth team members approve patients for the program. If appropriate, they also will refer patients to the NHP Care Team.
